OAD 26' Remembrance Video Submissions

Max’s Mission will be honoring lives lost to overdose and fentanyl poisoning with a remembrance video at Overdose Awareness Day 2026. Each year, this video is created as a community memorial, a space for families, friends, and loved ones to remember the people they have lost and to share the love, grief, and stories that continue to live on.

If you would like your loved one to be included in the memorial video, please email your submission to: OverdoseMemorial@gmail.com.

You may include any of the following:

  • Name or nickname

  • Photos

  • Birth and death dates

  • A short message about their life

  • What their loss means to you

  • Anything else you would like the community to know or remember

Submissions may come from family members, friends, or anyone who wishes to honor someone they have lost. Our goal is to create a meaningful remembrance that reflects the lives, love, and stories behind each name. Because Max’s Mission will be holding separate Overdose Awareness Day events in Klamath County and Jackson County, there are two different submission deadlines for remembrance video content.
